The E6 application form is required for the application of your SSS ID. To be qualified, you must have at least
paid 1 monthly contribution to your SSS account. You must provide your present address here in the UK
as it would be printed on your SSS ID.
Weight and height conversion charts are available for download in case you have any difficulties providing your
height and weight. You can view these charts by clicking on the links below.
Submit your application form by post. We will email you as soon as possible upon recieving your application form.
We will issue a date in which you should be in our office for biometric authentication. However, if you are not available
on a specific date, you can contact us to negotiate for a date of your preference.
Upon approval of the date of your appointment, you can refer to our website to check whether if you are scheduled
for appointment for your SSS ID.
Before you proceed to our office, please bring the following requirements:
- Passport and an additional ID
